DUTIES AND RESPONSIBILITIES:
The Individual Placement & Support Case Manager will serve as a case manager for employment support in the SEKMHC catchment area. This position is a case manager position with a focus on supported employment. The goal will be to connect the patients we serve with jobs, trade
programs, volunteer opportunities, and other resources that match their needs.
Individual Placement & Support:
1. Work with patients with severe and persistent mental illness to assist them in choosing, obtaining, and retaining employment in the community.
2. Coordinate supported employment activities, documentation, treatment, program planning, and goal setting for patients.
3. Provide information to the treatment team who have questions regarding patient issues, community resources, etc., while ensuring the treatment provided to patients is in accordance with SEKMHC policies and procedures.
4. Review goals and targets with the IPS Coordinator and/or the Clinical Director/CCBHC Grant Oversight to ensure that direct service goals are being met and develop plans to help the team achieve.
TARGETED CASE MANAGEMENT (TCM)
The goal is to enhance the patient's independent functioning by assisting them in obtaining access to needed medical, social, educational, and other services in accordance with their individualized treatment plan. Supports include:
1. Treatment Planning:
i. Facilitating or coordinating the team treatment planning process to include patients, treating therapists, families, and other natural supports.
ii. Assist in developing strengths-based goals and objectives, documenting the treatment plan, monitoring its effectiveness, and making changes when needed.
iii. Monitor treatment plan due dates, advise patient and therapist of due date, and schedule appointment for patient to meet with therapist to update plan prior to expiration date.
2. Collateral Contacts: Coordinate the patient's treatment needs via phone and written correspondence, as well as face-to-face contacts with other social service agencies, housing and employment resources, and medical services.
3. Access to Supports: Assist the patient in obtaining access to needed medical, social, educational, and other services. Also assist with applications for benefits and arrange transportation to needed services.
OTHER SERVICES:
Provide and/or coordinate these additional services as determined by the patient's individualized treatment plan:
1. Attendant Care
2. Psychosocial Rehabilitation Services
3. Crisis Intervention or Stabilization
DOCUMENTATION:
Document services in accordance with state, federal, and
organizational regulations, policies, and procedures.
1. All direct services and coordination of services must be documented in the patient's Electronic Health Record (EHR).
2. Document services concurrently (i.e., simultaneous to services) and submit to EHR in accordance with policies, procedures, and time requirements.
3. Complete and submit the month-end Patient Status Reports
REQUIRED TRAININGS:
Examples include:
1. Community Psychiatric Support and Treatment (CPST):
i. University of Kansas Basic Case Management Training (2 days) within 6 months after date hired.
ii. University of Kansas Advanced Case Management Training (1 per year). Attend within the first year of employment, and one training is required each year thereafter.
2. Adult Attendant Care Online Training (Track B): Complete prior to delivery of service.
3. Targeted Case Management Online Training: Complete within 60 days of hire and prior to providing service.
4. Crisis Intervention (CI) online training: Complete prior to delivery of service.
OTHER IN-HOUSE OR ONLINE TRAININGS:
As determined by your supervisor. Core trainings are due within the first year, and employees are advised by their supervisors when trainings are due. Examples include: RELIAS (https://sekmhc.training.reliaslearning.com/)
DIRECT SERVICE EXPECTATIONS:
This position has a minimum service credit expectation and will be provided by your supervisor.
TRANSPORTATION AND DELIVERY:
For work-related travel, employees must operate company or personal vehicles responsibly, in accordance with all traffic laws, regulations, and organizational safety policies.
WORKING CONDITIONS AND REQUIREMENTS:
1. Regular and punctual attendance is required and is an essential function of this position.
2. This position requires the ability to operate a vehicle as part of routine job functions.
3. This position requires the ability to use a computer and software as part of routine job functions.
4. Employees are expected to perform duties at the designated work location stated in the offer letter

QUALIFICATIONS:
Must be at least 18 years old and have a high school diploma or equivalent. Must meet the defined Kansas Department for Aging and Disability Services Behavioral Health requirements; must self-identify as a person with lived experience in raising a child with a Serious Emotional Disturbance (SED), Substance Use Disorder (SUD), or co-occurring mental health diagnosis. Must pass KBI, DCF child & adult abuse check, motor vehicle screen, and background checks. If the position requires the use of a personal vehicle for work purposes, the employee must have reliable, insured transportation that is properly registered and compliant with all applicable state and local requirements.

DUTIES AND RESPONSIBILITIES:
Parent Peer Support is a service that provides support to parents with children who are struggling with SED (Serious Emotional Disturbance), SUD (Substance Use Disorder), or co-occurring conditions and is provided to support the stabilization of the patient and enhance the family’s quality of life.
1. Use personal lived experience to offer hope, understanding, and encouragement to parents/caregivers.
2. Provide individual and group peer support to parents and families engaged in mental health services.
3. Assist families in navigating service systems, including mental health, education, child welfare, juvenile justice, and community resources.
4. Support families in advocating for their children’s needs and developing effective communication with
providers and systems.
5. Collaborate with clinical staff, case managers, and other professionals to promote family-driven, youth guided care.
6. Help parents develop skills in self-care, problem-solving, and crisis planning.
7. Maintain appropriate documentation of services in accordance with agency and funding guidelines.
8. Participate in training, supervision, and team meetings to enhance skills and maintain certification.
9. Uphold confidentiality and ethical standards consistent with peer support values and agency policy
REQUIRED TRAININGS:
1. LEVEL 1: Training toward becoming a Kansas Certified Parent Peer Support Specialist in Training (KCPPSST). IEP training through Families Together, following completion of Level 1 training.
2. LEVEL 2: Training occurs after KCPPSST certification and a 90-day supervision period

DUTIES AND RESPONSIBILITIES:
Our SEKA Children’s Aide program provides support to children within a collaborative team setting. This role requires the ability to transport patients using your personal vehicle, as needed and at the direction of your supervisor.
1. Turn in all staff daily service logs and roll call sheets directly to office staff via packet,
email, or fax as appropriate.
2. Service log and roll call sheets to ensure they are correct and match.
3. Check service logs and roll call sheets to ensure they are correct and match.
4. Review units and times on the logs to ensure they are correct and match.
5. Review and lock all staff progress notes in a timely manner as directed by policy.
6. Ensure narrative part of the note meets quality standard.
7. Ensure that appropriate reference numbers are included for each day, confirmation
code, units, time of service, and location are added as applicable.
8. Ensure that on each progress note, the assigned therapist's name for each child is
included on the note to be written in the top right-hand corner.
9. Meet with any staff who need additional training.
10. Assist with weekly shopping.
11. Share responsibility in all preparation required for group each day.
